How Do I Obtain My Immunization Record?

If you know the name of your  previous healthcare provider, that would be the best place to start.  Your  doctor should have a record of the immunizations given. This record could be transferred to a current healthcare provider or county health department and any additional vaccines needed could be administered.

Florida has a statewide, computerized immunization registry, Florida SHOTS™, and vaccination records for county health department clients are available (to authorized system users only) through a central database. The registry also includes the immunization records from many private physicians. Healthcare providers who are authorized immunization registry users may be able to access the records for any previous immunization in the state.

If you are unable to contact a previous healthcare provider, check with the county health department for the state and county from which you have moved. If that state has an immunization registry, you may be able to obtain a complete record from that state’s health department.

If none of those avenues prove successful, you should contact your private healthcare provider or county health department for further assistance. Explain the situation to them, and they will be able to assist you following proper medical protocol.

State of Georgia

There are several options you may choose from in searching for your record:

  1. There may be a record of your immunizations in GRITS, the Georgia Registry of Immunization Transactions and Services. The registry is not all-inclusive or comprehensive but may be helpful. To request this information, call 1-888-223-8644; FAX your request to 404-657-7496; or send an email toimmreg@dhr.state.ga.us. Identification will be requested and you will need to have this information available.
  2. Contact the health care provider who administered your last immunizations and request your record from them.
  3. Call the last school you attended to see if they still have your immunization certificate on file.
  4. If you had your immunizations administered in a public health clinic in Georgia, contact the County Health Department in the county the clinic was located in.
